Release Forms
Get permission from Individuals, Groups and Locations to appear in your production with these Release Forms. These forms are presented as templates for producers to use on their own productions.
Producers should collect authorization from individuals and locations appearing in their videos and keep them for their own records. These should not be turned in to CMNtv.
Individual Release Form
Use this release form for each person who appears in your production.
Group Release Form
Use this release form for large numbers of people who appear prominently in your production. Example: a featured studio audience, a large choir etc.
Location Release Form
Use this release form to gain written permission by a property owner for their location to appear in your production.
